Scenarios for problem-solving activity for the postgraduate students of public health dentistry

| Brief Introduction | |  |
Most of the times, we undertake problem-solving activity for the rural population only. However, it is important to train oneself in planning programs for different classes of population and in different settings. There is a difference in the manner in which we address the rural setting versus the urban setting. Here is a compilation of five different scenarios, which are unlike the routine problem situation that you may have solved earlier. These concentrate on urban setting, economics, and long-term evaluation of the program.

- Read the given scenarios carefully
- Concentrate on what information is given and how you can utilize it
- Enlist the missing data
- Think beyond the box. Incorporate the modern technologies for providing health education
- Urban settings have their own unique challenges. Hence, before proceeding, account for all those, for example, different oral health habits unlike the rural population, higher literacy level, access to all type of media, and also access to the dental facility
- Make use of the economic tools and evaluation tools for solving the problem.
Scenario 1
An urban-gated community of 30,000 flats is occupied majorly by the software employees. The population is of a metropolitan nature. The population comprises children: 15%, young adults: 50%, adults: 25%, and the geriatrics: 7%. The maintenance staff, housekeeping staff, and security staff comprise 3% of the total population.
There exists a community health clinic with different specialties, and assuming that the public health dentist is appointed to take care of the oral health status of this population, there arises a question of what are the strategies which need to be considered in this scenario?
Carefully analyze the scenario and plan a comprehensive oral health program for this population. You will be judged for your skills utilized in planning the program, understanding the scenario, recommendation suggested based on your knowledge of the modern technology, economics, and preventive, curative, and maintenance strategies.
Scenario 2
A community of Uber drivers who have work shifts of 12 hr per day have the habit of using smokeless tobacco. The drivers are in the age range of 22–45 years. A majority are literate to a minimum of 12th standard. With their job requiring them to drive cautiously and with full attention, they have indulged in this habit which according to them keeps them alert. The need of the hour is the tobacco cessation program for the driver community. Carefully analyze the scenario and plan a long-term program for the overall oral health benefit of the drivers. You will be judged for your skills utilized in planning the program, understanding the scenario, recommendation suggested based on your knowledge of the modern technology, economics, and preventive, curative, and maintenance strategies.
Scenario 3
Mahalunge village of Mulshi taluka is situated in Pune, Maharashtra. There are two Zilla Parishad schools. (one is a primary school with an anganwadi center in the same premise and the other is a secondary-level school). The agricultural land in the village is sold to the builders and urban living also exists in the village. There are two English medium private schools and shopping complexes. The village data are as follows.
One dental clinic and one private medical clinic exist in the village. Government hospital is 28 km away and primary health center is 15 km away. One dental college is 25 km away. The people living here are mainly workers. Tobacco and alcohol habit is prevalent.
Carefully analyze the scenario and plan a comprehensive oral health program. You will be judged for your skills utilized in planning the program, understanding the scenario, recommendation suggested based on your knowledge of the modern technology, economics, and preventive, curative, and maintenance strategies.
Scenario 4
All the nursery schools (n = 35) in the city are enrolled for a toothbrushing program, with a total of 14,000 children. The children in the nursery were in the age range of 3–5 years. You are appointed as a program director. Carefully analyze the scenario. Your duties include planning, execution, and evaluation of the program at 5 and 10 years' interval. Furthermore, prepare a budget for the same. You will be judged for your skills utilized in planning the program, understanding the scenario, economics, logistics, workforce planning, evaluation tools used, and execution skills.
Scenario 5
Pune district has 14 tehsils and 1912 villages. The total population of Pune is 9,426,959 and the current population of Pune urban agglomerate is 5 million (Census 2011). The male-to-female ratio is 1.09:1. The literacy level is 83.3%. There is no oral health data available for Pune district, however presented below are the published data from one of the villages in Pune. Assume that these data are representative of all the villages in Pune.

The oral health status was as follows: children at 5 years – 2.06 carious teeth per person and adults – 1.69 carious teeth per person. The treatment need for fillings is 65%.
6.9% edentulous population, precancerous lesion and conditions found in 14.88% population. The use of smokeless tobacco is highly prevalent.
As for as oral hygiene habit, 85% of the population are cleaning their teeth once per day using brush and paste. Pockets and calculus were present in majority of the adults.
Carefully analyze the scenario. Your duties include planning a comprehensive program at the family, village, and overall district levels. Questions such as what are the primary public oral health measures for the village population of Pune district and what are the strategies for execution and evaluation of the program at 10 and 20 years' interval need to be raised. Furthermore, prepare a budget for the same.
